Description
Right lateral contours outline the inferior (diaphragmatic) surface of the heart with the great vessels removed to keep attention on myocardial form. The right ventricle occupies most of this inferior surface, while the left ventricle contributes posterolaterally, and the inferior border runs obliquely toward the apex. Along the posterior aspect, the atrioventricular groove and the interventricular sulcus can be appreciated as shallow surface landmarks even without the coronary arteries and cardiac veins. Relationships to the diaphragm are implied by the flattened diaphragmatic impression. Clinically, this projection helps when you need to teach or plan around the heart’s contact surface with the central tendon of the diaphragm and adjacent pericardium. Inferior wall myocardial infarction typically involves the right coronary artery (or a left circumflex dominant pattern), and correlating ECG leads II, III, and aVF with the diaphragmatic surface improves anatomical reasoning even when the coronary vessels are not drawn. Pericardial effusions and pericardiocentesis discussions also benefit from a clean myocardial silhouette, since inferior pericardial recesses and the diaphragmatic pericardium are easier to conceptualize without vascular clutter.
