Description
Viewed from the right, the inferior border (margo inferior) of the heart is defined as a clean contour without the usual masking effect of the great vessels. Most of this sharp margin is formed by the right ventricle, with the diaphragmatic surface sloping posteriorly to meet the base. Near the acute angle at the junction of the sternocostal and diaphragmatic surfaces, the right atrium contributes superiorly, and the left ventricle begins to appear toward the leftward continuation of the diaphragmatic surface. No vessels distract the outline. Stripping away the accompanying vessels forces attention onto a landmark clinicians still describe at the bedside: the acute margin separating the anterior surface from the diaphragmatic surface, a contour that changes with right ventricular dilation and with pericardial effusion. It also maps directly to what you expect on frontal chest radiographs and axial CT, where the inferior cardiac silhouette rests on the central tendon of the diaphragm and can become blunted when the right ventricle enlarges. Surface anatomy matters.
