Description
Oriented from an inferior perspective, the heart’s diaphragmatic (inferior) surface is presented with the great vessels removed, leaving the ventricular contours and the obtuse and acute margins as the dominant landmarks. The left ventricle forms most of the left and posterior portion of this surface, while the right ventricle contributes more anteriorly and to the right, meeting at the interventricular groove. The apex lies anteroinferior and leftward, and the base is directed posterosuperior toward the atrial region. Clean and uncluttered. Removal of the vessels makes this view useful for teaching cardiac orientation and chamber contribution without the visual noise of the aorta, pulmonary trunk, and caval inflow. This is the surface that rests on the central tendon of the diaphragm, so it frames discussions of pericardiophrenic relationships and why inferior wall myocardial infarction (most often RCA territory in right-dominant circulation) can refer pain to the epigastrium or be accompanied by bradyarrhythmias from AV nodal ischemia. It also supports operative planning concepts for the transdiaphragmatic and posterior pericardial approaches, where the diaphragmatic pericardium and the inferior cardiac surface are the immediate field.
