Oriented from a posterior field of view, the inferior (diaphragmatic) surface of the heart is dominated by ventricular myocardium, with the left ventricle forming most of the undersurface and the right ventricle contributing anteriorly toward the acute margin. The posterior interventricular sulcus marks the boundary between the ventricles as it descends toward the apex, while the coronary sulcus separates the inferior atrial region from the ventricular mass superiorly. Atrial contours are present but subdued from this angle. No vessels are included. Seeing the diaphragmatic surface without associated veins and coronary arteries puts the muscular architecture and surface topography first, which helps when you need to teach how cardiac orientation relates to the central tendon of the diaphragm and the pericardium. This is the surface implicated in inferior myocardial infarction, most often from right coronary artery occlusion, where the adjacent posterior interventricular region and inferoseptal myocardium are at risk and where right ventricular involvement can change hemodynamic management.
