Description
Anteriorly, the adult female urinary bladder is opened in section to emphasize the circular smooth muscle layer of the detrusor vesicae forming a continuous ringlike coat deep to the vesical mucosa. The lumen lies centrally, with the circular fibers encircling it and blending imperceptibly with inner and outer detrusor bundles rather than forming a discrete sphincter. Superiorly the dome (apex) transitions toward the urachus region, while inferiorly the bladder neck narrows toward the urethra; laterally the muscular wall thickens toward the vesical margins where it relates to the pelvic sidewall and endopelvic fascia. Mucosal folds (rugae) may be suggested where the bladder is not fully distended, and the trigone region is implied posteroinferiorly even if not the primary focus. Seeing the circular detrusor layer in an anterior section matters when you need to explain how coordinated smooth muscle contraction reduces vesical volume during micturition without relying on an anatomically separate internal sphincter in the female.
