Description
Anteriorly sectioned adult female urinary bladder wall is opened to expose the detrusor vesicae, with emphasis on the internal longitudinal layer immediately deep to the urothelium and lamina propria. Muscle bundles run predominantly superoinferiorly and curve around the lumen, lying medial to the adventitia/serosa and anterior to the posterior bladder wall where the outer longitudinal and middle circular components are more apparent. Superiorly, the dome (apex) transitions toward peritoneal covering, while inferiorly the wall thickens toward the bladder neck leading into the proximal urethra. Separating the detrusor’s internal longitudinal fibers from the middle circular layer clarifies how coordinated contraction produces low-pressure emptying and why disruption alters compliance. This cut is useful when teaching female bladder outlet anatomy, where bladder neck dysfunction and iatrogenic denervation after pelvic surgery can present as urinary retention, and where distinguishing true detrusor underactivity from outlet obstruction affects management. Trabeculation from chronic outlet obstruction also makes sense here, since hypertrophy and reorientation of longitudinal bundles can exaggerate mucosal folding and produce sacculation. Urogynecology and urology courses use this perspective to discuss detrusor architecture, bladder compliance, and the relationship of the bladder neck to the female urethra, and it also fits atlases or review chapters on lower urinary tract physiology and urodynamics.
