Description
Anterior sectional anatomy of an adult female urinary bladder is presented with the detrusor vesicae forming the thick muscular wall deep to the mucosa and immediately external to the vesical lumen. Superiorly, the bladder dome (apex) rounds toward the peritoneal reflection, while inferiorly the detrusor fibers converge toward the bladder neck continuous with the internal urethral orifice. Medially, the trigone region sits at the bladder base between the expected locations of the ureteric orifices, and laterally the muscular wall maintains a uniform contour as it wraps around the organ. Smooth muscle bundles are the story here. Detrusor anatomy matters because its coordinated contraction and sphincter relaxation generate micturition, and small changes in muscle thickness and compliance underlie common urogynaecologic complaints. Detrusor overactivity drives urgency and urge incontinence, while impaired contractility contributes to urinary retention, often seen with diabetic autonomic neuropathy or after pelvic surgery.
