Description
Posteriorly oriented anatomy of the left lung base centers on the anteromedial basal segmental bronchus (bronchus segmentalis basalis anteromedialis), traced from the inferior lobar bronchus as it courses anteriorly and medially within the lower lobe parenchyma. The left main bronchus descends inferolaterally from the tracheal bifurcation, giving off the superior lobar bronchus before continuing as the inferior lobar bronchus toward the diaphragmatic surface. Segmental branch points are read from posterior to anterior, with the anteromedial basal segment turning away from the viewer as it approaches the anteromedial aspect of the basal pyramid. Orientation matters. Accurate identification of basal segmental bronchi underpins left lower lobe segmentectomy and guides bronchoscopy when targeting a peripheral lesion in the anteromedial basal segment (often labeled B7 plus B8 in older schemes because left-sided bronchopulmonary segments commonly show fusion or variable subdivision). This posterior viewpoint mirrors how the bronchial tree is mentally reconstructed when correlating bronchoscopy findings with CT, since distal basal bronchi run anterior to the accompanying pulmonary arteries while the pulmonary veins track more intersegmentally. Misreading that relationship can cost time in the OR.
