Description
Arising from the left lower lobe bronchus, the medial basal segmental bronchus (B7) courses inferomedially within the lower lobe, directed toward the mediastinal surface and the diaphragmatic base. Proximally, its lumen is continuous with the lobar bronchus and its cartilaginous wall transitions distally toward smaller segmental and subsegmental branches; the mucosa lines the airway centrally, with the bronchial wall encircling it. Medial to its expected trajectory lie the pericardium and esophagus, while laterally it is separated from adjacent basal segments by intersegmental connective tissue planes. Orientation emphasizes a ventromedial, left-sided relationship within the bronchopulmonary tree. For bronchoscopy and thoracic surgery, B7 matters because the medial basal segment sits close to the inferior pulmonary ligament and mediastinal pleura, and its takeoff can be a point of confusion when navigating the left lower lobe. Secretions and blood often track into dependent basal segments in supine patients, so recognizing the basal segmental bronchi helps explain aspiration patterns and guides targeted suctioning. Anatomical variants occur, including a common basal trunk or absent B7 with redistribution to B8 or B10, a detail that can change a planned segmentectomy margin. Small airway, big consequences.
