Description
Arising from the right inferior lobar bronchus, the medial basal segmental bronchus (B7) courses inferomedially toward the posteromedial portion of the lower lobe, adjacent to the mediastinal surface and just inferior to the hilum. Its lumen and cartilaginous wall are continuous proximally with the bronchus intermedius, while distally it divides into subsegmental bronchi that arborize within the medial basal segment (segmentum basale mediale). Medial to its distal course, the bronchus relates to the pericardium and right atrium, with the inferior pulmonary vein running anteroinferior at the hilum and the pulmonary artery more superior. Segmental bronchial anatomy matters because the right lower lobe often behaves as a set of discrete drainage units, and B7 sits in a position where aspirated material can track in the supine patient into dependent basal segments. This is the bronchus you want clearly in mind when planning segment-directed bronchoscopy, interpreting air bronchograms in medial basal atelectasis, or correlating an endobronchial lesion with a wedge-shaped opacity on CT. It also frames surgical thinking: segmentectomy and bronchoplastic approaches rely on recognizing B7 and separating it from neighboring basal bronchi, with attention to accompanying segmental pulmonary arterial branches and venous drainage along intersegmental planes.
