Description
Anterior sectional anatomy of the adult female urinary bladder highlights the muscularis propria (detrusor muscle) encircling the bladder lumen, with mucosa and submucosa lying internal to it and perivesical connective tissue external to it. The bladder neck tapers inferiorly toward the internal urethral orifice, continuous with the proximal urethra, while the dome (apex) sits superiorly and anteriorly in the pelvis. Muscle bundles interlace in multiple directions, but the overall wall thickening remains concentric around the cavity. Orientation is clear. Detrusor architecture matters when you teach continence and voiding: parasympathetic efferents (pelvic splanchnic nerves, S2 to S4) drive coordinated contraction while the bladder outlet relaxes, and disruption of that coupling is the substrate for neurogenic bladder. This view also supports interpretation of bladder outlet obstruction, where chronic overactivity and hypertrophy of the muscular layer can present as trabeculation and reduced compliance, contributing to urgency and incomplete emptying. Surgeons and urogynaecologists reference the bladder neck and anterior wall during anti-incontinence procedures and pelvic floor repairs, where dissection planes must respect the bladder wall to avoid perforation.
