Description
Along the lateral uterine wall, the uterine artery courses medially within the broad ligament to reach the cervix and uterine body, where it turns superiorly and ascends in a tortuous path toward the fundus. Branches radiate anteriorly and posteriorly through the myometrium, giving rise to arcuate arteries that run circumferentially, then to radial arteries that pass inward toward the endometrium. Near the endometrial interface, the arteries elongate into coiled spiral arteries that project toward the uterine cavity, while uterine veins accompany the arterial tree in a closely paired, plexiform arrangement. Ovarian anastomoses sit superior-laterally. This external profile matters because it ties surface landmarks to the functional layers that bleed, heal, and remodel with each cycle and across pregnancy. Spiral artery remodeling, driven by trophoblast invasion in early gestation, sits at the center of preeclampsia and fetal growth restriction when transformation fails, and the illustration makes the transition from arcuate to radial to spiral arteries easy to teach without resorting to histologic sections. For operative planning, the ascending uterine artery’s proximity to the ureter near the cervix (water under the bridge) remains a common point of injury during hysterectomy and uterine artery ligation.
