Description
Anterior sectioning through the adult female urinary bladder brings the bladder neck into focus, with the detrusor vesicae (muscularis of the bladder wall) thickening as it approaches the proximal urethra. Mucosa and submucosa line the lumen centrally, while the muscle bundles wrap circumferentially and blend inferiorly with periurethral smooth muscle at the internal urethral orifice. Superior to the neck, the bladder base and trigonal region sit posteroinferior to the dome, and the urethral lumen continues inferiorly in the midline, anterior to the vaginal wall and inferior to the bladder base. Attention to the detrusor region at the bladder neck matters when you are teaching continence mechanics and when you are planning interventions for bladder outlet symptoms in women. Detrusor overactivity and impaired bladder neck relaxation can coexist, and the relationship between detrusor fibers and the proximal urethra helps explain why urge incontinence, voiding dysfunction, and postoperative urinary retention are not interchangeable problems. A key surgical landmark. During anti-incontinence procedures and periurethral injections, small deviations in the bladder neck and urethral axis can change outlet resistance and alter risk of de novo urgency.
