Description
Viewed from an inferior perspective, the pancreas is presented with its head nestled in the C loop of the duodenum on the right and the body extending leftward toward the splenic hilum, tapering into the tail. The inferior margin helps separate the facies anterior pancreatis from the facies posterior pancreatis (dorsal surface), while the uncinate process projects posteriorly and medially from the head. Along the posterior aspect, the gland’s relationship to the posterior abdominal wall is implied by the contour where major retroperitoneal vessels typically course. Orientation is clear. Teaching the pancreatic surfaces from below matters because the “dorsal” surface is not an abstract label, it is the surface that lies against structures you injure when the dissection plane strays deep. During a pancreaticoduodenectomy (Whipple procedure), mobilization of the pancreatic head and uncinate process brings the surgeon into close proximity with the superior mesenteric vessels and portal confluence, and appreciating where the posterior surface transitions at the inferior border helps frame safe retroperitoneal exposure. Inferior inspection also reinforces why posterior pancreatic perforation can track into the lesser sac or retroperitoneum, a pattern often reflected in pancreatitis-related collections on CT.
