Lateral Presentation of the Stomach's Anterior Wall

Obliquely from the lateral side, the anterior wall of the stomach (gaster, ventriculus) is presented as a continuous gastric surface spanning the fundus superiorly, the body centrally, and the antrum and pyloric region distally toward the duodenum. The greater curvature sweeps inferiorly and laterally, while the lesser curvature turns superiorly and medially toward the lesser omentum. Expect the cardiac region near the gastroesophageal junction to sit posteromedial to the anterior wall contour, with the pylorus positioned more anterior and to the right in standard anatomical orientation. Orientation of the stomach’s anterior surface matters when you are thinking about what can be reached from an anterior abdominal approach and what cannot. Penetrating anterior gastric ulcers can perforate into the peritoneal cavity and present with free intraperitoneal air, while posterior wall ulcers more often erode into the lesser sac and can involve the pancreas or the splenic artery, a different clinical problem with a different operative and endoscopic mindset. A clear lateral presentation also helps explain why gastric distension or hiatal hernia can change the apparent relationship between the fundus and the left hemidiaphragm on imaging.
