Seen from the lateral aspect of the orbit, the globe sits anteriorly while the optic nerve (cranial nerve II) courses posteriorly from the optic disc toward the optic canal, contained within its meningeal sheath. Dura mater forms the outermost optic nerve sheath, continuous proximally with the cranial dura, while arachnoid and pia mater invest the nerve more intimately and define a subarachnoid space around it. The sheath lies medial to the lateral orbital wall and tracks toward the orbital apex, where the nerve enters the optic canal in the lesser wing of the sphenoid. Surrounding orbital bones frame the pathway, including the roof and floor of the orbit and the adjacent nasal cavity margins seen in profile. That dural sleeve matters clinically because it transmits intracranial pressure to the orbit, explaining papilledema and optic disc edema in raised intracranial pressure and giving the anatomic basis for optic nerve sheath diameter measurements on ultrasound or MRI. Compression at the orbital apex or within the optic canal can compromise both the optic nerve and its pial vascular supply, a pattern recognized in traumatic optic neuropathy and compressive lesions such as sphenoid wing meningioma. A clear lateral view also supports teaching of why optic neuritis pain often worsens with eye movement, the sheath and septa interact mechanically with the extraocular muscle cone even when the pathology is demyelinating rather than mass effect. Small spaces, big consequences.
