Description
Arcing across the basal surfaces, the margo inferior of the right and left lungs is rendered in color from an inferior perspective, with the diaphragmatic edge sweeping anterolaterally toward the acute margin. Laterally, that acute margin narrows as it approaches the costodiaphragmatic recess, while the more medial inferior border curves toward the region where the pericardium and mediastinal pleura would lie. Right-left asymmetry is implied at the base, with the right lung typically reaching a slightly lower level than the left along the midclavicular line. Attention to the inferior border matters when you are teaching pleural reflections and why the pleural cavity extends beyond the lung. In pleural effusion, fluid layers dependently into the costodiaphragmatic recess, blunting the costophrenic angle on upright chest radiographs and shifting the target for thoracentesis to a site that avoids the lung edge, commonly one to two intercostal spaces below the inferior border. Small errors in surface anatomy here puncture lung.
