Description
Posteriorly oriented lungs are rendered with emphasis on the inferior (lower, basal) lobes, which occupy the caudal third of each hemithorax and taper toward the diaphragmatic surface. The oblique fissure (fissura obliqua) forms a clear superolateral boundary, separating the inferior lobes from the superior lobes on both sides, while the left lung remains bilobed in keeping with normal anatomy. Medially, the paravertebral margins of the lungs align with the thoracic vertebral column, and laterally the basal segments sweep toward the costophrenic recess. Teaching the inferior lobes from a posterior perspective matters because gravity-dependent pathology collects here in the upright patient, and the posterior basal segments are a frequent site for aspiration pneumonia and dependent atelectasis. Bronchopulmonary segment orientation also becomes intuitive in this view, since the lower lobe bronchi course posteroinferiorly and their segmental distribution helps explain why a posterior lower-lobe opacity on chest radiograph or CT often localizes to the superior segment (S6) or the posterior basal segment (S10).
