Description
Paired inferior lobes of the right and left lungs are presented as the dominant basal divisions of the thoracic viscera, expanding inferiorly toward the diaphragmatic surface and tapering superiorly into their oblique fissures. Medially, the lower lobes wrap around the posterior mediastinum and descend toward the pulmonary ligament, while laterally they contour to the costal pleura. On the right, the inferior lobe lies inferior to the horizontal fissure and posterior to the middle lobe; on the left, it sits inferior to the superior lobe and posterior to the cardiac impression. Big territory. Lower lobe anatomy matters because so much clinical decision-making is gravity and bronchial anatomy in disguise. Basal (inferior) segment bronchi and their accompanying pulmonary arteries direct aspirated material to dependent regions, so right lower lobe bronchopneumonia and aspiration pneumonitis remain common patterns on chest radiography and CT. Surgeons also plan a lower lobectomy or segmentectomy around the oblique fissure, the inferior pulmonary vein at the hilum, and the relationship of the diaphragmatic surface to the phrenic nerve and pericardiophrenic vessels. A clean mental map reduces surprises.
