Description
Visible are the inferior lobes (lobi inferiores) of the right and left lungs, with their broad diaphragmatic (basal) surfaces conforming to the domes of the muscular diaphragm. Each lower lobe extends from the oblique fissure superiorly down to the costodiaphragmatic recess inferiorly, tapering anteriorly while expanding posteriorly toward the vertebral column. The right inferior lobe lies posteroinferior to the right middle lobe, while the left inferior lobe occupies the posteroinferior hemithorax beneath the superior lobe and lingula. Posterior segments sit closest to the paravertebral gutter. This emphasis on the basal lung is the teaching view you want when correlating lobar anatomy with gravity-dependent disease, because aspiration pneumonia and atelectasis tend to track into the posterior basal segments of the lower lobes in supine patients. Pleural relationships also read cleanly here: diaphragmatic excursion, pleural effusion layering, and the costophrenic angle all relate directly to how the inferior lobes rest on the diaphragm. Thoracostomy placement depends on that anatomy.
