Description
Colored segmentation isolates the right and left inferior (lower) lobes of the lungs in an anterior view, with the lobar masses distinguished from the superior lobes by clear color boundaries. On the right, the horizontal and oblique fissures define the inferior lobe posteriorly and inferiorly, while on the left the oblique fissure separates the inferior lobe from the superior lobe and lingula. Basal segments occupy the diaphragmatic surface inferiorly, and the medial aspects approach the mediastinum and cardiac silhouette. Size asymmetry is apparent. The right lower lobe typically has greater volume. An anterior emphasis on the inferior lobes matters because many dependent processes settle in the basal segments, including aspiration pneumonitis and atelectasis in supine or postoperative patients. Clinicians correlate these regions with auscultation and percussion points low on the posterior thorax, but the anterior projection helps learners reconcile why lower lobe pathology can still present with anterior findings near the costophrenic recesses. Radiology teaching benefits too, since lower lobe consolidation, pleural effusion layering, and blunting of the costophrenic angle can be traced back to these inferior, basal territories.
