Description
Posterior basal lung parenchyma dominates the inferior lobes, with segmenta basalia posteriora positioned posteroinferiorly and adjacent to the thoracic vertebral column. From a posterior orientation, the lower lobes sit inferior to the oblique fissures and extend toward the costodiaphragmatic recess, while their medial surfaces approach the paravertebral gutters. Segmental boundaries imply the course of segmental bronchi and pulmonary arteries that track centrally, then radiate distally toward the pleural surface. Depth matters here. For clinicians, the posterior basal segments matter because gravity, ventilation patterns, and patient positioning concentrate disease in the dependent lower lung. Aspiration pneumonia and atelectasis often localize to the posterior basal lower lobes in supine patients, and this segmental anatomy also guides bronchoscopy, CT reporting, and targeted physiotherapy. During thoracic ultrasound or pleural procedures performed posterolaterally, understanding how close these basal segments lie to the diaphragm and posterior chest wall helps interpret consolidation versus pleural effusion and avoid an unintended transdiaphragmatic trajectory.
