Description
Viewed in transverse section through the aortic root, the aortic valve sits centrally within the left ventricular outflow tract, with the right coronary leaflet (right coronary cusp, valvula semilunaris aortae) forming one margin of the tri-leaflet ring. The cusp attaches along its semilunar hinge line to the aortic annulus, and its free edge and nodule project toward the lumen, oriented to coapt with the adjacent noncoronary and left coronary leaflets. Just superior to the leaflet, the right aortic sinus (sinus aortae) expands laterally, and the proximal ascending aorta lies immediately cranial to the valve plane. Orientation of the right coronary leaflet matters because it defines the right coronary sinus and the ostial relationship of the right coronary artery, a frequent point of reference in aortic root surgery and transcatheter aortic valve replacement planning. Malposition of a prosthesis, bulky cusp calcification, or low coronary height can narrow the sinus or threaten coronary obstruction, and the right cusp is also a common site for degenerative calcific aortic stenosis that restricts systolic opening. Clear cusp identification also helps when teaching valve coaptation and regurgitant jet direction in aortic insufficiency.
