Description
Cut through the thorax in a transverse section, the aortic root sits centrally within the left ventricular outflow tract, with the three semilunar cusps (valvulae semilunares aortae) forming a triradiate closure line in systole and a tri-leaflet pocket configuration in diastole. The valve annulus is framed inferiorly by left ventricular myocardium and superiorly by the sinuses of Valsalva, where the right and left coronary ostia typically open into the respective aortic sinuses. Anterior and slightly rightward lies the right ventricular outflow tract and pulmonary trunk, while the left atrium and mitral valve complex sit more posteriorly in most standard transverse levels through the base of the heart. Spatial relationships are clear. This transverse perspective matters when you need to teach or recognize how aortic cusp geometry relates to flow, regurgitant orifice shape, and coronary filling during diastole. In calcific aortic stenosis, commissural fusion and cusp thickening reduce the effective valve area and create an eccentric jet into the ascending aorta, a pattern often correlated with post-stenotic dilation on imaging. The same anatomy guides transcatheter aortic valve replacement planning, where annular sizing, sinus dimensions, and coronary ostial height determine prosthesis choice and the risk of coronary obstruction.
