Description
Posteriorly oriented, this anatomical outline centers on the pyloric part of the stomach (gaster, ventriculus), including the pyloric antrum transitioning into the pyloric canal and ending at the pylorus adjacent to the proximal duodenum. The dorsal surface of the distal stomach lies anterior to the pancreas, while the pylorus sits to the right of midline and slightly inferior to the gastroesophageal junction. Along the curvatures, the lesser curvature runs superior and medial toward the hepatoduodenal region, and the greater curvature sweeps inferior and lateral into the left upper abdomen. A posterior view is the one you want when teaching relationships that matter in the operating room: the pylorus and first part of the duodenum form the anterior boundary of the epiploic (omental) foramen, immediately anterior to the portal triad in the hepatoduodenal ligament. Peptic ulcer disease at the posterior wall of the pyloric canal or duodenal bulb risks bleeding from the gastroduodenal artery, a complication that is easier to conceptualize when the dorsal gastric surface is mentally placed against the pancreatic head. Short. This orientation also supports discussion of pyloric hypertrophy and gastric outlet obstruction by clarifying the muscular “gate” at the gastroduodenal junction.
