Description
Seen from behind, the stomach (gaster, ventriculus) is invested by a smooth gastric serosa that remains continuous over the fundus and body before reflecting along the greater and lesser curvatures. Superiorly the serosal coat approaches the gastroesophageal junction and abdominal segment of the esophagus, while inferiorly it continues onto the antrum and pylorus. Medially, the serosa conforms to the lesser curvature where the lesser omentum attaches, and laterally it drapes over the greater curvature toward the gastrosplenic and gastrocolic ligaments. A thin, uninterrupted peritoneal envelope is the point. Posterior serosal anatomy matters because the posterior wall of the stomach relates directly to the lesser sac, so peritoneal reflections define the operative planes encountered during mobilization of the stomach and access to the pancreas. In perforated posterior gastric ulcers, contamination often tracks into the lesser sac rather than the greater peritoneal cavity, changing early clinical signs and the pattern of fluid seen at CT or during diagnostic laparoscopy. Clear depiction of the serosal continuity also supports teaching of why most of the stomach is intraperitoneal, while the attachment lines at the curvatures tether it through the omenta.
