Description
Sectioned at a transverse plane through the right ventricular outflow tract, the pulmonary trunk sits anterior and slightly leftward relative to the ascending aorta, with the pulmonary valve interposed at their junction. Dominating the field is the right semilunar leaflet (cuspis semilunaris dextra), its concave surface facing inferiorly toward the right ventricular infundibulum and its convex arterial surface directed superiorly into the pulmonary trunk. The commissures bracket the cusp laterally, while the free margin and central nodule align toward the lumen, defining the line of coaptation with the adjacent anterior and left leaflets. Orientation like this matters when you need to teach or interpret valve mechanics rather than just label parts. Pulmonic stenosis, whether congenital (often with dome-shaped leaflets and commissural fusion) or acquired, changes the geometry of the cusp belly and the coaptation line, and that geometry drives Doppler gradients and post-stenotic dilatation of the main pulmonary artery. Surgeons and interventional cardiologists also think in these relationships during pulmonic valvotomy, transcatheter pulmonary valve implantation, and reconstruction of the right ventricular outflow tract in tetralogy of Fallot repairs. Small structure, big consequences.
