Seen in lateral profile, the rectal ampulla forms a capacious dilation of the distal rectum, continuous superiorly with the rectum proper and tapering inferiorly into the anal canal. Anterior to the ampulla in the adult male, the bladder base, seminal vesicles, and prostate lie in close apposition, separated by the rectovesical fascia. Posteriorly, the ampullary wall relates to the sacrum and coccyx through the presacral space, while the peritoneal reflection and rectovesical pouch sit superior and anterior to the upper rectum. Clinical work in the male pelvis often hinges on this segment. The ampulla distends as a fecal reservoir, so its caliber and compliance influence constipation patterns, fecal impaction, and the mechanics of defecation.
