Description
Shown in female pelvic anatomy, the uterine corpus sits superior to the cervix, which projects inferiorly into the vaginal vault and is continuous laterally with the parametrial connective tissues at the level of the internal os. Stage 2 cervical cancer is represented as an infiltrative cervical carcinoma extending beyond the cervix and uterus, with spread toward the upper vagina and or medial parametrial tissues while remaining short of the pelvic wall. The endocervical canal, ectocervix, and surrounding cervical stroma form the central anatomic focus, with the tumor mass expanding outward from the transformation zone into adjacent soft tissue planes. Stage II disease carries specific staging implications under FIGO: II A indicates involvement limited to the upper two thirds of the vagina without parametrial invasion, whereas II B indicates parametrial invasion, and that distinction changes both prognosis and primary treatment selection. MRI staging hinges on the integrity of the hypointense cervical stromal ring and the presence of spiculated or nodular tumor signal extending into the parametrium, a pattern that correlates with higher risk of lymphovascular space invasion and nodal metastasis. Small differences in lateral spread matter. They separate candidates for radical hysterectomy from those more often managed with definitive chemoradiation and brachytherapy planning.
