Description
Centered in the female pelvis, the uterine corpus tapers inferiorly into the cervix, whose ectocervix projects into the superior vagina while the endocervical canal continues proximally toward the endometrial cavity. An irregular cervical carcinoma expands laterally into the parametrial tissues adjacent to the uterine arteries and toward the pelvic sidewall, with inferior spread along the vaginal cuff toward the lower third of the vagina. Posteriorly, the lesion approaches the rectovaginal septum and anterior rectal wall, while anterior extension tracks toward the vesicouterine space and bladder base. Regional lymphatic drainage follows the paracervical channels to obturator, internal iliac, and external iliac nodal basins. Stage 3 cervical cancer matters because the pattern of spread changes both staging and treatment planning in a way that is specific to the cervix. FIGO stage III includes tumor reaching the pelvic wall, involving the lower third of the vagina, and or causing hydronephrosis or a nonfunctioning kidney from ureteric encasement, so depiction of the ureters as they cross beneath the uterine artery near the cardinal ligament is a key teaching point. Expect symptoms tied to this anatomy: pelvic pain from pelvic sidewall involvement, obstructive uropathy, and abnormal bleeding from friable cervical tissue.
