Description
Shown in profile, the uterine cervix sits inferior to the uterine corpus and projects into the superior vagina, with the endocervical canal running from the internal os superiorly to the external os inferiorly. Stage II cervical carcinoma is represented as tumor extending beyond the cervix into the upper vagina and or parametrial tissues, while remaining medial to the pelvic wall and sparing the lower third of the vagina. Anteriorly the cervix relates to the bladder base and vesicouterine space, and posteriorly to the rectum across the rectouterine pouch. Boundaries matter. FIGO stage II is where local spread begins to change management, because parametrial invasion (stage IIB) predicts lower likelihood of achieving clear margins with radical hysterectomy and often shifts care toward definitive chemoradiation. Distinguishing IIA (limited to the upper two thirds of the vagina without parametrial involvement) from IIB depends on appreciating the lateral cervix and parametrium, including the proximity of the ureter as it courses beneath the uterine artery near the cervicouterine junction. Pelvic MRI is commonly used to assess stromal depth and parametrial extension, and this anatomical framing matches what radiologists and gynecologic oncologists look for when correlating examination findings with imaging.
