Viewed from a superior perspective, the paired lungs are organized into bronchopulmonary segments with emphasis on the anterior segments: the anterior segment of the right upper lobe (segmentum anterius, S3) and the anterior segment of the left upper lobe (S3), with the left lung also contextualized by the lingular segments inferiorly. Segmental boundaries correspond to the distribution of segmental bronchi and accompanying branches of the pulmonary artery, while pulmonary veins course in the intersegmental planes. From this angle, the anterior segments occupy the anterolateral portion of the upper lobes, lying anterior to the more posteriorly placed apical and posterior segments and superior to the middle lobe (right) or lingula (left). Clear fissural landmarks often include the oblique fissures bilaterally and the horizontal fissure on the right. Anterior segment anatomy matters whenever disease localizes to an anterosuperior distribution, as in aspiration in supine patients, where dependent portions shift and anterior upper lobe segments may be involved depending on positioning. Segment-level understanding also guides bronchoscopy and anatomic segmentectomy, since S3 can be targeted while preserving adjacent parenchyma, and surgeons rely on intersegmental venous drainage to define a safe plane of division. Small errors in segment identification can translate into missed margins or unexpected air leaks. Precision counts.
