Description
Seen from a superior aspect, the left upper lobe bronchial tree is oriented around the lingular segmental bronchus (bronchus segmentalis lingularis), branching anteroinferiorly toward the lingula from the upper lobe bronchus. Secondary divisions extend distally into smaller bronchi and bronchioles, with the lingular pathway lying anterior and lateral relative to more posterior segmental branches of the upper lobe. The segmental caliber is still apparent at the origin, while the subsequent arborization narrows quickly into conducting bronchioles. Clean airway geometry. Understanding the superior orientation of the lingular segment helps clarify why lingular disease can mimic right middle lobe pathology on radiographs, both being anterior, gravity-dependent segments when patients are supine. This angle also matches the bronchoscopy operator’s mental map when advancing from the left main bronchus into the left upper lobe bronchus and selecting the lingular segmental orifices for bronchoalveolar lavage, culture, or endobronchial biopsy. Segmental anatomy matters in postobstructive atelectasis as well, where a mucus plug or aspirated material can selectively collapse the lingula while sparing adjacent upper lobe segments.
