Description
Seen from a superior perspective into the left hemithorax, the superior (apical) segmental bronchus of the left lower lobe (B6) is traced as it branches from the left inferior lobar bronchus and courses posterolaterally toward the apex of the lower lobe. Its segmental divisions radiate within the superior segment, lying inferior to the upper lobe bronchus and superior to the basal segmental bronchi that supply the lower lobe’s inferior regions. Proximal bronchial cartilage rings and the central bronchial lumen define the airway pathway before it arborizes distally. Orientation is everything. Understanding the top down orientation of B6 matters in bronchoscopy and anatomic lung resection, where distinguishing the superior segment of the lower lobe from the posterior segment of the upper lobe can prevent wrong segment navigation or incomplete clearance. In left lower lobe superior segmentectomy, the surgeon identifies and divides the B6 bronchus in close company with the A6 segmental pulmonary artery branch and corresponding intersegmental planes, a relationship that becomes easier to teach when the bronchial axis is viewed from above. This segment is also a common site of dependent infection or aspiration related changes on CT, where mucus plugging and tree in bud opacities often follow segmental bronchial distribution. Faculty can drop this figure into thoracic anatomy lectures, bronchoscopy orientation modules, or operative atlases covering VATS and robotic segmentectomy of the left lower lobe, and radiology educators can pair it with axial CT slices to reinforce segmental localization.
