Description
Arising from the left lower lobar bronchus, the medial basal segmental bronchus (B7) is presented from a superior perspective as it courses inferomedially within the lower lobe toward the diaphragmatic surface. Its lumen lies medial to the more lateral basal segmental branches (anterior and lateral basal) and sits in close relation to the posterior basal bronchus as the basal segments fan out around the hilum. From above, the bronchial pathway can be appreciated heading toward the central tendon region of the diaphragm, with adjacent pulmonary parenchyma framing the segmental boundaries. For bronchoscopy and thoracic surgery, B7 is a common source of orientation errors because it angles medially and can appear short or partially hidden behind neighboring basal bronchi when approached from the main carina. Mucus plugging, aspiration, or localized infection can track into basal segments, and the medial basal segment often demands targeted suctioning or selective intubation when managing lower lobe atelectasis. Segmentectomy planning also depends on recognizing this bronchus to avoid incomplete resection or injury to adjacent segmental vasculature running in parallel within the bronchovascular bundle.
