Description
Seen in left lateral orientation, the medial basal segmental bronchus (B7) of the left lower lobe branches from the lower lobar bronchus and courses inferomedially toward the basal segments. Its lumen narrows as it runs distally, with a short, tapered segment before it arborizes into smaller subsegmental bronchi within the pulmonary parenchyma. Superior to this basal takeoff, the path of the superior segmental bronchus (B6) is implied, while adjacent basal bronchi (B8 anterior, B9 lateral, B10 posterior) would lie in close anatomic neighborhood even if not all are fully profiled in a strict lateral view. Small airway, big consequences. Segmental bronchial anatomy matters when disease respects segmental boundaries. The medial basal segment can be selectively obstructed by mucus plugging or an aspirated foreign body, producing basilar atelectasis that may be subtle on frontal radiographs but better appreciated on lateral imaging or CT, where the relationship of B7 to the lower lobe bronchial tree guides interpretation. Bronchoscopic navigation and thoracoscopic segmentectomy also depend on identifying B7 correctly, because the left lower lobe often shows anatomic variation and crowding of basal bronchi near the hilum, increasing the risk of wrong-segment entry or stapling. Pulmonology teaching files, thoracic surgery atlases, and radiology correlation chapters use this type of focused airway plate to anchor segmental nomenclature and to pair bronchial branching with typical distributions of aspiration, bronchiectasis, and localized collapse.
