Description

Seen from the anterior aspect, the anterior interventricular artery (left anterior descending, LAD) descends in the anterior interventricular sulcus from the left coronary artery, running inferiorly toward the apex between the right and left ventricles. Proximally, the ascending aorta sits superior and posterior to the coronary origins, with the pulmonary trunk anterior and slightly leftward at the base. Diagonal branches course obliquely over the anterior surface of the left ventricle, while septal perforators dive posteriorly into the interventricular septum; the great cardiac vein typically parallels the LAD in the same groove, superficial and slightly left of the artery. This anterior mapping matters because the LAD supplies much of the anterior left ventricular wall and the anterior two thirds of the interventricular septum, territories that often declare themselves in anteroseptal ST elevation myocardial infarction. Short takeoff length from the left main coronary artery, the relationship to the left atrial appendage, and the course relative to the pulmonary trunk are practical details when planning coronary angiography, CT coronary angiography interpretation, or surgical revascularization. A frequent teaching point is dominance and collateral flow, but this view keeps the focus where most occlusions happen.
