Seen from the anterior aspect, the anterior interventricular artery (left anterior descending, LAD) descends in the anterior interventricular sulcus from the left coronary artery, running inferiorly toward the apex between the right and left ventricles. Proximally, the ascending aorta sits superior and posterior to the coronary origins, with the pulmonary trunk anterior and slightly leftward at the base. Diagonal branches course obliquely over the anterior surface of the left ventricle, while septal perforators dive posteriorly into the interventricular septum; the great cardiac vein typically parallels the LAD in the same groove, superficial and slightly left of the artery. This anterior mapping matters because the LAD supplies much of the anterior left ventricular wall and the anterior two thirds of the interventricular septum, territories that often declare themselves in anteroseptal ST elevation myocardial infarction.
