Emerging from the left coronary artery, the anterior interventricular artery (left anterior descending, LAD) courses inferiorly in the anterior interventricular sulcus toward the apex, running between the right and left ventricles in close company with the great cardiac vein. From an inferior perspective, the vessel’s distal segment and its relationship to the apical region come forward, while proximal landmarks such as the left coronary cusp region of the ascending aorta and the pulmonary trunk sit superior and posterior relative to the ventricular mass. Diagonal branches angle laterally over the anterolateral left ventricle, and anterior septal perforators dive posteriorly into the interventricular septum. Coronary venous tributaries in blue track back toward the coronary sinus. This angle matters because it clarifies how the LAD wraps toward the diaphragmatic surface near the apex, a region where coronary dominance and the length of the LAD influence which territories become ischemic when occlusion occurs. A short apical LAD with early diagonal takeoff looks different from a long wraparound LAD that reaches the inferior wall, and that distinction shapes ECG interpretation in anterior versus apical infarction patterns.
