Description
Semi-transparent lungs in an x-ray style frame the central tracheobronchial tree, with the trachea descending in the midline and bifurcating at the carina into the right and left main bronchi. Cartilage rings support the trachea and proximal bronchi as C-shaped arcs, while the more distal bronchi transition to discontinuous cartilage plates embedded in the bronchial wall. Branching continues inferiorly and laterally into lobar and segmental bronchi within each lung, tapering toward bronchioles where cartilage is no longer present. Attention to cartilage architecture matters because it explains why the airway behaves differently at each level of the conducting system. Intact hyaline cartilage prevents dynamic airway collapse during expiration and cough, while loss or softening of rings and plates, as in tracheobronchomalacia or post-intubation injury, produces wheeze, stridor, and air trapping that can mimic asthma.
