Description
Running along the dorsolateral aspect of the upper limb, the cephalic vein is traced from the dorsal venous network of the hand over the posterior wrist and forearm, then proximally along the lateral border of the radius toward the elbow. Proximal to the cubital fossa it continues on the lateral arm, superficial to the fascia, with the humerus lying deep and medial to its course. Radius remains lateral and ulna medial in anatomical position, a helpful orientation when following tributaries from the dorsum of the hand into larger superficial channels. Arterial counterparts, including the brachial artery and forearm branches, sit deeper and more protected. Posterior mapping of the cephalic vein matters because venous access and venous injury do not occur only on the anterior surface, and dorsal hand cannulation commonly recruits flow into the cephalic system. Its superficial course along the lateral forearm and arm makes it a frequent target for venipuncture, while its proximal trajectory toward the deltopectoral region underpins cephalic vein cutdown for device access when peripheral cannulation fails. A wide caliber is not just a drawing choice, it reflects why the vein is favored for PICC placement and why thrombophlebitis from repeated cannulation can become clinically obvious along a long, palpable segment.
