Semi-transparent soft tissues of the anterior thoracoabdominal wall reveal the alimentary tract of an adult Asian woman in profile, tracking the esophagus as it descends posterior to the trachea, passes through the esophageal hiatus of the diaphragm, and joins the stomach in the left upper abdomen. The liver occupies the right hypochondrium and overlies the gastric antrum and proximal duodenum, with the gallbladder tucked on its visceral (inferior) surface, while the pancreas sits posterior to the stomach along the C-loop of the duodenum. Centrally, jejunum and ileum coil within the peritoneal cavity, and the large intestine frames them peripherally, continuing distally into the rectum along the posterior pelvis. Relationships read clearly. A lateral orientation matters when you need to teach depth, not just topology: the stomach and pancreas sit posterior to the anterior abdominal wall, and the esophagus runs in close posterior relationship to the airway before entering the abdomen. This is the setup for common clinical discussions, from gastroesophageal reflux disease and hiatal hernia at the gastroesophageal junction, to pancreatitis presenting with epigastric pain that can radiate posteriorly because of the gland’s retroperitoneal position.
