Description
Seen in lateral profile, the middle rectal vein courses from the lateral wall of the rectum through the pararectal connective tissue toward the pelvic sidewall, where it drains into the internal iliac vein. Anteriorly, its pelvic segment lies in relation to the urinary bladder and prostate, while posteriorly it approaches the sacrum and anterior surface of the vertebral column. Superiorly the rectum continues into the sigmoid colon, and inferiorly the anal canal descends toward the perineum, with the venous channel positioned medial to the bony pelvis and deep to the pelvic floor musculature. Small tributaries from the rectal venous plexus are implied along its proximal course. This venous pathway matters because the middle rectal vein forms a key systemic outflow route from the rectum, distinct from the superior rectal vein (portal drainage via the inferior mesenteric vein) and the inferior rectal veins (to the internal pudendal system). That anatomical separation underpins patterns of spread and bleeding: lower rectal tumors tend to metastasize to internal iliac nodes and beyond, and surgical dissection during total mesorectal excision or low anterior resection often encounters troublesome venous branches tethering the mesorectum to the lateral pelvic wall. Expect variation. The middle rectal vein may be paired, small, or absent, which is exactly why a clear lateral relationship to the internal iliac vein is worth having on hand.
