Description
Running vertically on either side of the anterior abdominal wall, the paired rectus abdominis muscles extend from the pubic crest and pubic symphysis inferiorly to the xiphoid process and the costal cartilages of ribs 5 to 7 superiorly. Tendinous intersections segment each muscle belly into distinct bellies, separated in the midline by the linea alba. Superiorly, the rectus lies immediately inferior to the costal margin and rib cage; inferiorly it approaches the anterior pelvis, with the vertebral column implied posterior to the thoracoabdominal framework. Clinically, this anterior view is the clearest way to teach the rectus sheath concept, because the rectus abdominis is enclosed by the aponeuroses of the external oblique, internal oblique, and transversus abdominis, and those layers reorganize at the arcuate line to create a weaker posterior wall below it. That detail matters in abdominal wall hernias and in operative planning for midline laparotomy, paramedian incisions, and rectus-splitting approaches. It also grounds discussion of rectus diastasis, where widening of the linea alba alters abdominal wall mechanics without a true fascial defect. A common exam pitfall.
