Posterior thigh anatomy is centered on the biceps femoris, with the short head (caput breve) lying deep and lateral to the long head (caput longum) as it descends toward the knee. The short head arises from the lateral lip of the linea aspera and lateral supracondylar line of the femur, while the long head originates proximally from the ischial tuberosity, so the two bellies converge distally into a common tendon. Along the lateral knee, that tendon inserts on the head of the fibula, with the fibula positioned lateral to the tibia and the patella shown anterior to the femoral condyles. Gluteus medius is visible on the lateral ilium, superior to the proximal femur. Clear landmarks. This relationship matters because the short head is the only hamstring component that does not cross the hip joint, so weakness or injury can present as isolated deficits in knee flexion and lateral rotation without the hip extension findings you expect from long head involvement. Distally, the biceps femoris forms the superolateral border of the popliteal fossa, and its tendon sits close to the common fibular (peroneal) nerve as the nerve winds around the fibular neck, a frequent site of traction or compression during lateral knee trauma and proximal fibular fractures. Surgical approaches to the posterolateral corner and repairs near the fibular head rely on this spacing and on identifying the fibular collateral ligament and biceps tendon as distinct layers.
