Description
Presented in an anterior-oriented vertical cross section, the male kidney is opened to expose the renal cortex and medulla with their pyramids converging on the renal papillae and draining into calyces and the renal pelvis, which continues inferiorly as the ureter. A lobulated malignant tumor expands from the parenchyma, effacing corticomedullary anatomy and bulging beyond the renal capsule. Superiorly, the mass extends toward the ipsilateral adrenal gland and adjacent perirenal tissues, consistent with advanced local invasion. Stage four renal malignancy is most often discussed clinically in the context of renal cell carcinoma with extracapsular extension, regional involvement, and a tendency for vascular spread via the renal vein into the inferior vena cava. That anatomy matters in the operating room: radical nephrectomy planning hinges on whether tumor breaches Gerota fascia, reaches the adrenal, or tracks medially toward the renal hilum where the artery, vein, lymphatics, and collecting system crowd together. Advanced disease also explains obstructive features, with distortion of the pelvis and proximal ureter contributing to hydronephrosis and flank pain, while nephron loss in the cortex and medulla drives declining renal function.
