Cricothyroid Membrane

Description

Anterior laryngeal anatomy centers on the cricothyroid membrane spanning the interval between the inferior border of the thyroid cartilage superiorly and the arch of the cricoid cartilage inferiorly. The midline thickening forms the median cricothyroid ligament, while the paired lateral cricothyroid membranes blend posterolaterally with the conus elasticus (cricovocal membrane) that continues superiorly toward the vocal ligaments. Superior to this level, the epiglottis rises behind the thyrohyoid region, and inferiorly the first tracheal rings align in series beneath the cricoid. Sex-specific surface contour is implied by the male thyroid prominence and a narrower anterior cricothyroid interval compared with the surrounding cartilaginous framework. Access at the cricothyroid membrane matters because it is the fastest, most direct route into the infraglottic airway in a “can’t intubate, can’t oxygenate” scenario. Palpation of the thyroid notch, thyroid laminae, and cricoid arch guides cricothyrotomy needle or scalpel placement while avoiding lateral deviation toward the cricothyroid muscle and superior thyroid vessels. This is a small target. The relationship to the conus elasticus also explains why an overly superior incision can violate the vocal folds and produce dysphonia or subglottic stenosis.
