Description
Centered in the anterior neck, the thyroid cartilage forms a shield-like framework superior to the cricoid cartilage, with the laryngeal prominence and paramedian laminae defining the midline. Spanning between the inferior border of the thyroid cartilage and the superior margin of the cricoid arch, the cricothyroid membrane (median cricothyroid ligament and adjacent conus elasticus) appears as a broad, sheet-like fibroelastic interval. Superiorly, the hyoid bone sits above the thyroid cartilage, while inferiorly the first tracheal rings continue caudally from the cricoid. Small paired infrahyoid strap muscles and the anterior bellies of the cricothyroid muscle typically border this region laterally, framing the membrane. An anterior view of the cricothyroid membrane matters because it is the fastest surface route into the infraglottic airway during an emergency cricothyrotomy. Midline placement minimizes bleeding by avoiding the cricothyroid arteries coursing more laterally and reduces the chance of injuring the vocal folds, which lie superior to the membrane at the level of the glottis. In adult males the membrane can be easier to palpate due to a more prominent thyroid notch and laryngeal prominence, but obesity, laryngeal trauma, or prior neck surgery can obscure the landmark.
