Description
Oriented as a transverse section from the lumen outward, the wall is organized into mucosa, submucosa, muscularis externa, and an outer serosa (or adventitia where retroperitoneal). The mucosa forms tall villi projecting into the lumen, with simple columnar epithelium overlying lamina propria and a distinct muscularis mucosae at its deep margin. Deep to this, the submucosa sits external to the muscularis mucosae and internal to the muscularis externa, with the inner circular layer medial to the outer longitudinal layer; the myenteric (Auerbach) plexus lies between those muscle layers, and the submucosal (Meissner) plexus runs within the submucosa. Layer order matters. This layered architecture underpins how the duodenum, jejunum, and ileum differ in both histology and clinical behavior. Brunner glands in the duodenal submucosa help distinguish proximal small bowel, while prominent plicae circulares and long villi point toward jejunum, and aggregated lymphoid nodules (Peyer patches) in the ileal mucosa and submucosa explain why terminal ileum commonly shows aphthous ulcers and transmural inflammation in Crohn disease. Surgical and endoscopic decisions hinge on what layer is involved, from mucosal celiac sprue changes to submucosal vascular lesions and muscularis hypertrophy in obstructive patterns.
