Description
Centered in the right upper quadrant of the torso, the gallbladder lies on the visceral (inferior) surface of the liver, tucked in its fossa between the right and quadrate lobes. The fundus projects anteroinferiorly toward the anterior abdominal wall, while the body courses posteriorly and medially to the neck, which narrows into the cystic duct. From the porta hepatis, the right and left hepatic ducts unite as the common hepatic duct, and the cystic duct joins it to form the common bile duct descending inferiorly toward the duodenum. A smooth serous coat covers the peritoneal surface, contrasting with the hepatic surface where the organ apposes liver parenchyma. Orientation matters. This relationship between liver, gallbladder, and extrahepatic biliary tree underpins the symptom pattern in biliary colic and acute cholecystitis, where an obstructing gallstone at the gallbladder neck or cystic duct produces right upper quadrant pain and can refer to the right shoulder via diaphragmatic irritation.
