Description

Seen from the lateral thoracic wall, the internal intercostal muscles (intercostales interni) occupy most of each intercostal space as red fiber bands spanning between adjacent costae. Their fibers run inferoposteriorly from the inferior border of the rib above toward the superior border of the rib below, lying deep to the external intercostals and superficial to the innermost intercostals. Posteriorly, the rib angles and the thoracic vertebral column provide orientation for where the muscular portion transitions toward membranous intercostal tissue. Bony landmarks dominate. Ribs, costal cartilages, and thoracic vertebrae frame the field. Functionally, this view clarifies how internal intercostals contribute to forced expiration by depressing the ribs and stabilizing intercostal spaces during active breathing and coughing. Their relationship to the neurovascular plane matters: the intercostal vein, artery, and nerve course between the internal and innermost intercostals, so a lateral depiction helps teach why tube thoracostomy and thoracentesis are directed just superior to the rib to avoid the main neurovascular bundle. Needle trajectory is not an academic detail. It is the difference between a safe pleural tap and an intercostal arterial bleed.
